How to Register

If you would like to register with the practice please call into the surgery to pick up a registration pack, or alternatively go to the "Downloads" area of our website and download the registration form GMS1 and our New Patient Questionnaire.   Please note failure to complete a New Patient Questionnaire may result in a delay in processing your registration.

You are free to consult the GP of your choice. You are a patient of the practice and registering you with a particular doctor is purely for administration purposes only.

As a condition of joining the practice we ask all new patients over the age of 10 to have a new patient medical. This is a 10 minute appointment with the practice nurse,  to which you will be asked to bring along a sample of urine. The receptionist will provide you with a sample pot for this purpose . If you are on repeat medication you will need to make an appointment with the doctor prior to you needing your next repeat prescription.
