Summary Care Record (SCR)

SCR is an electronic record of important information regarding your health. All patients over the age of 16 should have received a letter from NHS Bedfordshire explaining the process of SCR. At the moment the only information included in an SCR is current medication, allergies and adverse drug reactions. If you do not wish your SCR to be available to Health Care Professionals in other areas, you will need to let your practice know by completing an opt out form (see download area of the website). Patients under 16 will not be receiving a letter and an SCR will be automatically created unless the practice is advised otherwise.
